The way the world looks and sounds is completely different of the vanilla gameplay.

Some places look alien from the vanilla game. Places that used to be safe heavens are now death zones. Some areas, although may look similar, but plays completely different. The only group left is the small number of NCR troops and they shoot on sight since mostly everyone that aint NCR are psychopaths or cannibals.

There's no more factions since the plague is killing everyone. Just like Uylesse said, them damn tunnelers have invaded the mojave and they are everywhere now that they are left unchecked due to the plague. There's a plague(deadly clouds from Sierre Madre) in the air that's killed off most NPCs, and the ones thats survived, are drove insane that they attack anything on sight.(even you, the player are insane, with a insane perk) Some of them are even cannibals. The mod takes place 20 years after the events of New Vegas, and the mojave has turned into complete shit.

I must warn you, this mod is NOT for the casual, this is for the hardcore players that wants a challenge. your only objective is to survive and escape the mojave. What I mean by that is, you have no real objective here, there are no quest, or no friendly NPCs. Turning the game into, what would seem like, DayZ.
